This is an Act 5 Main quest, granted by Scorv Egdenor during the A Witch's Bargain quest.
The quest will differ depending on whether the player is Friendly or Hostile with Barrowholm.

If you became friendly with Barrowholm, Scorv will ask you to help two of Barrowholm's Villagers. Talk to Tyvald Hallows and Jordyth Evelline, and complete the Wendigo Wards and Travelers Beware side quests for them. Return to Scorv Egdenor's house and retrieve the Ugdenbog Fetish from his chest.
If you became hostile, defeat Scorv, Servant of Ravager and collect the Barrowholm Cellar Key quest item, which unlocks the Forlorn Cellar. This area leads to the Den of the Wendigo, where the Ugdenbog Fetish will drop from Namadea, the Reaper.
Return the quest item to Garradia to compete the quest.

| You met with Scorv Egdenor and agreed to assist the people of Barrowholm with their struggles in exchange for the Coven's ritual piece.
Speak to Tyvald Hallows or Jordyth Evelline in Barrowholm and assist them with whatever they require. Then Scorv will grant you what you seek within his stash. Once you have the Ugdenbog Fetish, return it to Garradia at the Coven's Refuge. Objectives:

| You met with Scorv Egdenor of Barrowholm, but refused to assist him, instead opting for a violent means to acquiring the Ugdenbog Fetish.
As the relic was clearly not within easy reach on the surface, you will have to check the cellar behind Scorv for what you need; but it is locked and Scorv likely has the key. Slay Scorv Egdenor in Barrowhold and take his cellar key in order to access the cellar. Once inside, find the Ugdenbog Fetish and return it to Garradia at the Coven's Refuge. Objectives:
